Take the first step toward improving or maintaining your health by getting to know the resources on this site. You can:
- Login to NC HealthSmart's Personal Health Portal where you can take a Personal Health Assessment (PHA) and receive a Personal Action Plan; explore condition centers, like back pain or diabetes; and learn about lifestyle programs, such as ways to improve your nutrition.
- Learn how to create a health-friendly workplace using our resources including NC HealthSmart Worksite Wellness and the Worksite Wellness Toolkit as an excellent tool.
- Review Your Preventive Care and Immunization Benefits – The State Health Plan is committed to providing preventive services at the lowest cost possible to help members lead healthy, happy and productive lives. Preventive care benefits include routine physical and screening procedures, laboratory tests and immunizations. Access the Preferred Provider Organization Preventive Services Benefits.
- Interested in quitting tobacco, losing weight, finding helpful tips on how to reduce stress or even how to start an exercise program? If you want some help with these or any health issue, visit our Wellness page.
- Discover ways to manage chronic diseases, like diabetes, asthma, high blood pressure or chronic kidney disease. Find help if you are experiencing multiple hospitalizations, a sudden catastrophic event or a high-risk pregnancy. - Call a Health Coach, specially trained professionals, such as nurses, respiratory therapists or dietitians, who are available to answer questions you may have concerning the health of you or your family.
